There supposedly a picture of a box of a little swiss knife sitting on the edge of his tub. A 2 to 3 inch knife can easily produce a 4 to 6 inch stab wound, maybe more. You have to remember about the compression of the body upon impact. Hitting the bag with your fist compresses the bag 2 to 3 inches allowing your fist to penetrate the plane of the bag several inches. Put a 2 to 3 inch blade at the end of your fist and the fist goes in 2 to 3 inches with the 2 to 3 inch blade penetrating 2 to 3 inches farther. Hence you now have a 4 to 6 inch hole in the son of a gun. Even more if you have a more powerful strike that penetrates deeper.
