You would be sent to the barbers (yep hairdressers!) who would be the local sawbones. To show that they weren't as important as doctors they would be addressed as "Mr". Over time the doctors who did become involved in the cutting end of the business retained the term Mr, so now you have a lower title used for a superior position!
