This quirk has been retained in UK only and possibly in some (ex) colonies (not Canada). All doctors start as "doctors" although strictly speaking none are doctors at all, just Bachelors of Medicine and Surgery expressed in various quaint and long abbreviations depending on the alma mater. It is only when they pass their postgraduate exams (FRCS in the case of the surgeons and obstetricians) are they allowed to call themselves "Mister" or "Miss", although I am not sure if the latter title has been changed to allow for political correctness (in my days you could count women surgeons on the fingers of one hand).
