If you look further back in history than what you have described, you will see that there were some different forms of pugilism where the participants would nominate the body parts that were to be used in the contest, Eg: Upper body only, feet and hands, full body. It is rather intersting to see how similiar those contests and the period of boxing would have been to MA.
