Let me first point out that there is one difference between person a trying to give Fluffy a bath and person b trying to pound your head into the pavement: Person a cares about Fluffy's well being. What this means is that even though person a is in a physically dominant position and able to inflict massive harm on Fluffy, they will tend to not do that since their aim is the cleaning and not the defeat of Fluffy. Imagine how docile Fluffy might get after all 4 legs get broken and her head gets held underwater for 2 minutes. (Note: I am not advocating this for Fluffy. I do not know Fluffy, but I wish her the best.) The woman who escaped from the trunk was a cargo that the attackers cared about. They wanted her for something, rather than just trying to beat her head in. This changes what tactics the attacker will use. The notion of going ballistic when on the ground has a certain amount of merit, but this is based on the assumption that the person on top has little experience in dealing with such tactics. I should point out that very experienced Brazilian Jiu Jitsu fighters are well prepared for this. Their tactic is to basically let you go ballistic. Why? Because you'll burn out your energy in 2-4 minutes and then be this blob of jelly that they can more easily subdue. All they have to do is: 1) keep you under them and 2) keep from getting hurt. Now, these seem generally hard to do, and it involves a lot of detailed positioning work. Your head stays here when they body is there, you watch for this limb from that position, etc. However, this is a technical skill that can be learned. All that is required is time and practice and your teeth and fingers will never find an effective target. Note that they don't have to pay attention to what happens to you while they are subduing you. So, keep thinking ballistic. It will work with a large percentage of attackers. However, bear in mind that for certain attackers, this will only hasten your end. Better to learn enough ground defense to give you a chance of getting back up again.
